RMA Request Form
Instruction:
1.All fields MUST be completed. By submitting this form, you agree to abide by all
Valor Multimedia warranty policies.
2.Please complete the RMA form with detailed description of the problem(s) for your
product. Product(s) without a problem description will delay the warranty process
completion. Use additional forms if necessary.
3.FAX your completed RMA form to our RMA Dept. at (626) 581-9980
4.The RMA Dept. will FAX back to you an RMA number within 24 hrs.
5.After you receive your RMA number, ship the returned product(s) to Valor Multimedia.
6.Make sure to include the RMA number on the outside of your shipping carton.
7.If your products are not returned within 14 business days of the issued date
of the RMA number, you must resubmit your request for a new RMA number.
